LICENSED SKYDIVER REGISTRATION AND INFORMATION FORM

Welcome and thank you for choosing to jump with Skydive Bay Of Islands (SBOI). We know that when you arrive at the dropzone you want to get into the sky as soon as possible so to make the check in process easier you can:

1. Read though this document.

2. Fill out the jumper information.

When you arrive at the DZ bring your in-date equipment, license, logbook and this document (we will have a copy for you too) and you can sign the waiver on site. Your logbook will be checked for experience/jump numbers and currency (see NZPIA currency requirements). Please note you will also need to complete the Sport Jumper Quiz questions on site.

NOTE: All skydivers jumping at a landing area close to the beach must carry a life vest in case of emergency water landing. You will be briefed on its use and our Water Landing Procedure.
